BEST PRACTICES FORUM: Transitional Care Program for the Elderly
January 8, 2009 - Orlando, FL - The Center for Health Futures hosted the Best Practices Forum on Transitional Care addressing the complex health care needs of the elderly. Eric A. Coleman, MD, MPH, Professor of Medicine and Director, Care Transitions Program at the University of Colorado Health Science Center, presented. The forum explored how essential information and services can fall between the cracks and how to reduce medical errors, improve health outcomes and avoid hospital readmissions among the elderly. Best practices in the coordination and continuity of healthcare for the elderly were discussed, with an emphasis on periods of transition between healthcare providers and family caregivers.
